Print out - Student Health Record Form 14 or obtain a copy from your child's school
Fill out the first “Student Section” with your child’s identifying information
Confirm that your child had a physical within the last year (365 days), which is required to fill out the form. If you are not sure of this, please check MyChart or call the office to confirm this before dropping off the form.
Drop off the form to our office during business hours. We will do our best to fill out the form at the time of drop off but it can take up to 3 business days if we are busy.
